Licensed Practical Nurse (LPN) Rehabilitation

Hospital Sisters Health System Rehabilitation Hospital is a brand-new 40-bed inpatient rehabilitation facility (IRF) seeking a Licensed Practical Nurse (LPN) to join our rehabilitation team. This is a full-time position working nights on a 3x12 schedule, with a sign-on bonus offered.

Clinical Setting

This role is part of Lifepoint Health, a diversified healthcare delivery network with facilities across the country. You will work in a modern inpatient rehabilitation setting focused on helping patients recover and transition successfully back to their communities.

Responsibilities

• Deliver direct and indirect patient care using the nursing process of Assessment, Planning, Intervention, and Evaluation
• Develop nursing care plans in coordination with patients, families, and interdisciplinary staff
• Monitor and communicate changes in patient clinical conditions to physicians, nursing supervisors, managers, and colleagues
• Collaborate with the interdisciplinary care team to create personalized care plans
• Participate in the discharge planning process to support safe, successful patient transitions

Schedule

• Full-time, nights
• 3x12-hour shifts

Required Qualifications

• Active Wisconsin or compact state LPN license required. If relocating, licensure must be updated to your new Primary State of Residence before employment begins.
• Associate's Degree
• AHA Basic Life Support (BLS) certification within 30 days of hire
• Critical thinking skills, decisive judgment, and ability to work with minimal supervision
• Ability to work in a stressful environment and take appropriate action

Preferred Qualifications

• One year of previous nursing experience
