Bethesda is seeking a Licensed Practical Nurse (LPN) to provide basic nursing care in a rehabilitation setting. In this full-time position, you will work 6:30am to 7:00pm, assisting in the assessment of residents on admission and on an ongoing basis. You will remain knowledgeable about significant changes in resident condition and communicate these to the interdisciplinary team. You will participate in care planning, implement nursing interventions, administer medications and treatments safely, and document all assessments and interventions accurately and timely. This role requires strong critical thinking skills to identify behavioral changes and respond to residents' evolving needs. You will monitor care delivery, answer call lights, assist with activities of daily living as needed, and maintain composure during crisis situations.

Responsibilities

  • Assist in assessing residents on admission and on an ongoing basis
  • Communicate significant changes in resident condition to the interdisciplinary team
  • Participate in the care planning process and update care plans when interventions indicate revision
  • Implement nursing interventions including medication administration and treatments in a safe and timely manner
  • Administer IV fluids if IV certified
  • Document all assessments and interventions accurately and timely
  • Use critical thinking skills when dealing with residents' changes in condition
  • Notify the physician timely using SBARR communication tool
  • Monitor care delivery to ensure the care plan is being implemented effectively
  • Identify problem areas and use the QAPI process for immediate problem resolution
  • Answer call lights and assist with residents' activities of daily living as needed
  • Complete a minimum of twelve hours of nursing-related continuing education
  • Maintain current knowledge of Bethesda's policies and best nursing practices

Required Qualifications

  • Education from an accredited school of nursing resulting in Licensed Practical Nurse credential
  • Current license in the state where you will be working as a Licensed Practical Nurse
  • IV Certification required or willing to obtain within six months of employment
  • Current CPR/AED Certification required or willing to obtain within six months of employment
  • Ability to assess the basic nursing care needs of geriatric residents
  • Excellent communication skills to interact with residents, visitors, and staff
  • Excellent time management and organizational skills
  • Critical thinking skills

Preferred Qualifications

  • Long-term care, rehabilitation, or assisted living experience
