Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse (LPN) to join our healthcare team and provide compassionate, high-quality care to individuals served. In this role, you will work hands-on supporting the nursing process—assessment, planning, implementation, and evaluation—while collaborating with a supportive team of healthcare professionals. You will help individuals feel cared for, safe, and supported while building your clinical skills in a mission-driven environment.

Responsibilities

• Assist in assessing individuals served physical, psychological, and social needs
• Contribute to individualized treatment planning with RNs and other team members
• Implement treatment plans, including diagnostic tests and patient teaching
• Administer medications safely, including IV therapy when certified
• Monitor and document patient care using SOAP charting
• Support minor medical or surgical procedures as directed
• Respond to health emergencies with confidence and professionalism
• Ensure controlled substances and medical supplies are properly counted and secured

Required Qualifications

• Graduation from an accredited school of nursing
• At least 1 year of acute medical experience
• Current, unrestricted LPN license in the state of practice or multi-state compact license
