Licensed Practical Nurse (LPN) - PRN

This is a PRN (as-needed) Licensed Practical Nurse position in a community-based residential setting serving residents who require ongoing nursing care and support.

Responsibilities

• Oversee medication distribution according to physician orders, ensuring strict adherence to Community medication management documentation and procedures while prioritizing resident safety
• Conduct thorough assessments and create personalized service plans tailored to individual care needs while supporting resident independence and dignity
• Facilitate communication between physicians, healthcare providers, and family members regarding residents' physical, mental, and social well-being
• Contribute to quality assurance and performance improvement initiatives
• Participate in continuous learning through attendance at educational seminars, staff meetings, and other professional development opportunities

Required Qualifications

• Current state licensure as Licensed Practical Nurse (LPN)
• Current CPR certification
• Exceptional communication and interpersonal skills
• Demonstrated ability to provide care with compassion and dignity
