Licensed Practical Nurse - LPN

Presidential Home Care LLC is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in the home health setting throughout the Marietta, GA area. In this role, you will deliver personalized, high-quality care to patients in their homes, working as part of a collaborative care team to support patients' health and independence.

Responsibilities

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Develop and follow individualized patient care plans in coordination with RNs and physicians
• Observe and document patient conditions, reporting changes to supervising clinical staff
• Assist patients with activities of daily living (ADLs) and personal hygiene as needed
• Educate patients and family members on care procedures, medications, and health management
• Maintain accurate and timely clinical documentation in compliance with state and agency standards
• Coordinate with the care team to ensure seamless, continuous home health services

Required Qualifications

• Active and unrestricted LPN license in the state of Georgia
• Strong knowledge of nursing procedures, medication protocols, and patient safety standards
• Excellent communication and interpersonal skills with patients and care teams
• Reliable transportation and ability to travel to patient homes in the Marietta, GA area
• CPR/BLS certification required or willingness to obtain
• Ability to work independently with minimal supervision
• Detail-oriented with strong organizational skills

Preferred Qualifications

• Prior experience in home health, long-term care, or a clinical nursing setting
