Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse (LPN) for full-time night shifts to provide direct patient care in a residential setting. This role requires genuine empathy, patience, and a caring approach to meet the needs of residents and support the nursing team.

Responsibilities

• Conduct patient assessments, follow established protocols, and report medical or behavioral changes
• Document clinical findings and maintain accurate medical records
• Perform admission duties including completing assessments, medication administration records (MAR), treatment administration records (TAR), plan of service (POS), reviewing chart setup and physician orders, and ordering medications
• Manage patient discharges by completing required documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care including following protocols, irrigating, packing, and removing dressings
• Lead certified nursing assistants (CNAs) and certified medication technicians (CMTs) while ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations

Required Qualifications

• Active LPN license
