Licensed Practical Nurse (LPN) - Part-time - 2nd shift

This part-time Licensed Practical Nurse position works the 2nd shift in a community-based residential setting, providing direct patient care and medication management for residents. You will oversee medication distribution according to physician orders while maintaining strict adherence to community medication management documentation and procedures, with a focus on resident safety.

Responsibilities

• Oversee medication distribution as stated on physician orders, ensuring strict adherence to community medication management documentation and procedures
• Conduct thorough assessments and create personalized service plans tailored to individual resident care needs
• Champion resident independence and dignity in all care interactions
• Cultivate communication between physicians, healthcare providers, and family members regarding residents' physical, mental, and social well-being
• Contribute to quality assurance and performance improvement initiatives
• Participate in continuous learning through attendance at educational seminars, staff meetings, and other learning opportunities

Required Qualifications

• Current state licensure as Licensed Practical Nurse (LPN)
• Current CPR certification
• Exceptional communication and interpersonal skills
• Ability to serve with compassion and dignity
