Licensed Practical Nurse (LPN), Park Ridge Living Center (Full Time, Days)

Park Ridge Living Center, a CMS 5-Star facility and part of Rochester Regional Health, is seeking a Licensed Practical Nurse (LPN) for full-time day shifts in our Transitional Care Center. You will provide professional patient care to long-term and post-acute care residents in our home-like Wegman Cottages or hospital-connected rehab unit. In this role, you will direct and monitor care provided by CNAs, ensure proper documentation, and lead your team in delivering quality individualized care.

Responsibilities

• Administer medications to assigned residents in a timely and accurate manner with concurrent required documentation
• Perform treatments and document progress according to physician orders and facility protocol
• Ensure concise, pertinent, and complete resident care documentation using the computerized medical record system
• Initiate appropriate care pathway tools
• Direct assigned CNAs in timely completion of quality individualized personal care in compliance with residents' individual care plans
• Ensure CNA documentation is accurate and complete
• Receive and give relevant reports, both written and verbal, to appropriate staff according to departmental protocols
• Contact physicians, transcribe orders, and follow up with appropriate action to institute orders in a timely manner
• Monitor residents and report relevant condition status changes to the charge nurse, supervisor, or physicians in a timely manner
• Complete pre-reviews of medication administration records according to facility protocols
• Complete required MDS documentation and all related observations in a timely manner
• Support the CNA team by assisting with direct ADL care of residents

Schedule

• Full-time position, 36 hours per week
• Day shift: 7:00 AM to 3:30 PM
• Every other weekend and holiday required

Required Qualifications

• Graduate from an accredited School of Practical Nursing
• Current LPN licensure in the State of New York
• Basic Life Support (BLS) Certification from the American Heart Association (AHA)

Preferred Qualifications

• One year of experience

Physical Requirements

• Medium work: exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
• Frequent walking, standing, or squatting required
