Licensed Practical Nurse / LPN or Registered Nurse / RN for Private Duty Nursing / PDN Case

Summit Home Care, a family-owned private duty nursing agency, is seeking Licensed Practical Nurses (LPNs) and Registered Nurses (RNs) for immediate openings throughout Bergen County. This role offers the opportunity to provide direct patient care in home settings, with positions available for both experienced nurses skilled in specialized care and new graduates eager to develop their skills through our PDN education program.

Clinical Setting

Private duty nursing in patients' homes with a focus on complex medical needs including tracheostomy care, ventilator management, and gastric tube (G-tube) care.

Responsibilities

• Follow the plan of care developed by the Nurse Manager
• Administer medications and treatments according to the established plan of care
• Monitor clients' conditions closely and report any changes promptly
• Maintain accurate documentation in accordance with provided orders

Required Qualifications

• Current nursing license in New Jersey (RN or LPN)
• Up-to-date First Aid and CPR certification
• Up-to-date medical documents
• Experience with tracheostomy, ventilator, and/or G-tube care
