Licensed Practical Nurse (LPN) - North Greece, NY - Days

Lifetime Assistance is seeking a Licensed Practical Nurse (LPN) to provide compassionate, person-centered health care to individuals with intellectual and developmental disabilities in residential settings. This full-time position is based in Rochester, NY, with responsibilities split between two locations (14626 and 14612).

Clinical Setting

You will work in residential settings supporting individuals with intellectual and developmental disabilities, reporting to the Residence Manager. The role involves direct patient care, medication administration, health monitoring, and coordination of medical services.

Responsibilities

• Deliver compassionate, person-centered health care to individuals in residential settings
• Administer medications and treatments as prescribed; monitor health status and report changes to the RN
• Provide personal care, support daily living routines, and promote independence
• Coordinate and accompany individuals to medical appointments; ensure follow-up care is carried out
• Assist the RN with developing nursing care plans, health-related goals, and staff training
• Respond to emergencies and behavioral incidents, utilizing SCIP-R strategies
• Act as shift leader when assigned and collaborate with interdisciplinary teams
• Maintain accurate documentation in compliance with agency and regulatory requirements
• Support a clean, safe, and welcoming home environment

Schedule

• Full-time, 40 hours per week
• Monday through Friday, 7:00 AM to 3:30 PM

Required Qualifications

• Current New York State Licensed Practical Nurse (LPN) license in good standing
• Valid New York State driver's license and ability to meet Lifetime Assistance's Vehicle Operator Requirements
• CPR and First Aid certification, or ability to obtain
• SCIP-R Level II certification, or willingness to complete training
• Strong communication, problem-solving, and decision-making skills
• Ability to follow medical directions and adapt to changing care needs
• Ability to lift up to 35 pounds and perform frequent physical activity
• Ability to respond to emergencies
