Licensed Practical Nurse (LPN)

Meadow Falls of Conneaut, a premier long-term care facility, is seeking a Licensed Practical Nurse (LPN) to provide direct patient care to residents. In this role, you will work every other weekend on 12-hour shifts, delivering compassionate care and collaborating with an interdisciplinary team to support resident health and well-being.

Responsibilities

• Provide direct patient care, including administering medications, performing treatments, and assisting with daily living activities
• Develop and implement individualized care plans in collaboration with the interdisciplinary care team
• Monitor and report changes in patient condition to the nursing staff and medical team
• Collaborate with other healthcare professionals to ensure comprehensive care and support
• Document patient information accurately and thoroughly in accordance with facility policies and procedures
• Participate in ongoing education and training to maintain current knowledge and skills

Required Qualifications

• Current LPN license
• Experience as an LPN
• Ability to work effectively in a fast-paced environment
• Strong communication and interpersonal skills

Schedule

• Every other weekend
• 12-hour shifts

Compensation and Bonus

• Competitive hourly rate of $28 to $30
• $2,500 sign-on bonus
