Licensed Practical Nurse (LPN - LVN) Rehabilitation

Texas Rehabilitation Hospital of Fort Worth is seeking a Licensed Practical Nurse (LPN) to join our 66-bed rehabilitation facility. In this role, you will provide direct patient care under the supervision of a Registered Nurse, delivering treatments, assisting in assessments, and supporting interdisciplinary care planning. You will work with patients throughout their rehabilitation stay, from admission through discharge.

Texas Rehabilitation Hospital of Fort Worth is part of Lifepoint Health, a diversified healthcare delivery network with acute care, rehabilitation, and behavioral health facilities. This position is located in Fort Worth, Texas.

Schedule

• Full-time position
• 3x12 hour shifts
• Night shift: 7:00 PM to 7:00 AM

Responsibilities

• Perform accurate patient assessments and identify patient needs
• Identify and initiate appropriate nursing interventions
• Provide direct patient care under RN supervision
• Deliver treatments and assist in assessments
• Support interdisciplinary care planning according to patient needs
• Provide patient and family teaching throughout the rehabilitation stay and prior to discharge
• Provide age-appropriate and condition-appropriate care
• Work cooperatively with interdisciplinary team members

Required Qualifications

• Graduate of an accredited licensed practical/vocational nursing program
• Current state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)
• CPR/BCLS certification
• Minimum one year of LPN experience

Preferred Qualifications

• ACLS certification
• IV certification (required within 6 months if not currently held)
• Six months of medical/surgical experience in an acute care setting
