Lincoln Village Healthcare Center is seeking a Licensed Practical Nurse (LPN) to provide nursing care to residents in our long-term care facility. Working under the supervision of a Registered Nurse, you will administer medications, monitor resident health status, and support care plans to promote optimal outcomes.

Clinical Setting

This position is located at Lincoln Village Healthcare Center in Lincoln, Illinois. You will work in a long-term care environment with frequent resident contact, exposure to body fluids, and potential exposure to infectious diseases. The role requires adherence to safety and infection control protocols and the ability to work collaboratively within a healthcare team.

Responsibilities

  • Administer medications and treatments as prescribed, in accordance with facility policies and state regulations
  • Monitor residents' health status, observe for changes in condition, and promptly report findings to the RN or physician
  • Assist residents with activities of daily living (ADLs) as needed
  • Ensure residents receive appropriate nutritional support and hydration
  • Accurately document all care provided, medications administered, and resident observations in the medical record
  • Participate in care plan development and interdisciplinary team meetings
  • Communicate effectively with residents, families, and healthcare team members
  • Follow infection prevention and control procedures, including proper use of personal protective equipment (PPE)
  • Maintain a safe environment for residents, visitors, and staff
  • Comply with all state, federal, and facility regulations, including HIPAA
  • Direct and monitor the work of Certified Nursing Assistants (CNAs) to ensure quality care
  • Provide training, guidance, and feedback to CNAs as needed
  • Assist in onboarding and orientation of new nursing staff

Required Qualifications

  • Current, valid Illinois LPN license in good standing
  • Graduation from an accredited practical nursing program
  • Strong clinical skills and ability to respond quickly in emergency situations
  • Excellent communication, leadership, and organizational skills
  • Ability to lift, push, and pull up to 50 lbs and assist with resident transfers
  • Frequent standing, walking, bending, and reaching
  • Ability to work in a fast-paced environment with potential exposure to infectious diseases

Preferred Qualifications

  • Prior experience in long-term care or a similar healthcare setting

Schedule and Shifts

This position offers 12-hour shifts (6 a.m. to 6 p.m. or 6 p.m. to 6 a.m.). All shifts are available. This is a non-exempt position.

Compensation and Benefits

Pay: $35.00 per hour
