Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse (LPN) to provide direct patient care in our facility. This position offers a $10,000 sign-on bonus and requires genuine empathy, patience, and a caring approach to supporting residents' needs. Success in this role depends on strong work ethic, integrity, and clinical competence.

Responsibilities

• Conduct patient assessments, follow established protocols, and report medical or behavioral changes
• Document clinical findings and maintain accurate medical records
• Perform admission duties including completing assessments, medication administration records (MAR), treatment administration records (TAR), plan of service (POS), reviewing chart setup and physician orders, and ordering medications
• Manage patient discharges by completing required documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care including following protocols, irrigating, packing, and removing dressings
• Lead and supervise CNAs and CMTs while ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
