Licensed Practical Nurse (LPN)

Peterson Park Health Care Center is seeking Licensed Practical Nurses to join our clinical team at our post-acute care facility located at 6141 N. Pulaski Road, Chicago, IL. As an LPN, you will serve as a backbone member of the Clinical Department, providing direct nursing care to residents in a facility specializing in post-hospital rehabilitation, wound care, short-term respite stays, and dedicated dementia and Alzheimer's care through our Evergreen Unit.

Clinical Setting

Peterson Park Health Care Center holds Gold Seal of Approval Certification for Post-Acute Care and Accreditation for Nursing Care from The Joint Commission. The facility is conveniently located in the family-friendly Peterson Park neighborhood on Chicago's Northwest Side with easy access to public transportation and major thoroughfares.

Responsibilities

• Ensure residents receive medication and nursing care as directed by their individual care plans
• Conduct resident assessments
• Serve as a team leader within the clinical department
• Advocate for residents
• Maintain high standards of resident comfort and safety through knowledge, skills, and attention to detail

Required Qualifications

• Unencumbered State of Illinois Licensed Practical Nurse (LPN) license

Preferred Qualifications

• Prior Licensed Practical Nurse experience
