Lakefront Nursing and Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) to join our clinical team at our facility located at 7618 N. Sheridan Road in Chicago's Rogers Park neighborhood. This is an opportunity to provide skilled nursing care in a community-focused setting that emphasizes rehabilitation, mental health programming, and helping residents reintegrate into independent living.

Clinical Setting

Lakefront Nursing and Rehabilitation Center is a skilled nursing facility with over 40 years of experience providing short-term and long-term care, rehabilitation services, and mental health programming. The facility serves a diverse population, including individuals dealing with addictions and other mental health conditions. We collaborate with numerous agencies throughout Chicago to ensure residents have access to comprehensive resources and support.

Responsibilities

As an LPN, you will be the backbone of our Clinical Department. Your responsibilities include:

  • Ensure residents receive medication and nursing care as directed by their individual care plans
  • Conduct resident assessments
  • Serve as a team leader
  • Advocate for residents
  • Maintain resident comfort and safety through knowledge, skills, and attention to detail
  • Provide high-quality care that meets the standards residents have come to expect at Lakefront

Required Qualifications

  • Unencumbered State of Illinois Licensed Practical Nurse (LPN) license

Preferred Qualifications

  • Prior LPN experience (we are willing to invest in the right candidate without prior experience)
