Licensed Practical Nurse LPN Home Health/Mobile Case Management position! Adults/Geriatric focused

This mobile case management position offers Licensed Practical Nurses an opportunity to expand beyond traditional bedside care. You will support adults and geriatric patients through a Transitions of Care program, focusing on care coordination, patient education, follow-up visits, and reducing hospital readmissions. You will work alongside an RN-led multidisciplinary team in a home health and mobile case management setting.

Responsibilities

• Coordinate care for patients transitioning through the healthcare system
• Provide patient education and support
• Conduct follow-up visits and assessments
• Work to reduce hospital readmissions
• Collaborate with an RN-led multidisciplinary team

Schedule

• Days, Monday through Friday
• 8:00 AM to 4:30 PM
• 5 shifts of 8 hours each
• 40 guaranteed hours per week
• No weekends
• 13-week assignment length

Required Qualifications

• Active New York LPN license
• Minimum of 1 year recent home health, case management, or clinic experience
