Licensed Practical Nurse (LPN) Home Health

Vertus HealthCare is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. In this role, you will enable patients to remain in their home environments by delivering professional nursing services, supporting patients and their families through illness, rehabilitation, and preventative care.

Responsibilities

• Provide skilled nursing interventions under the direction of a registered nurse
• Deliver professional nursing services in the treatment of patients' illnesses, rehabilitative needs, and preventative care
• Utilize a holistic approach in providing problem-specific therapeutic interventions, teaching, and training activities in accordance with the individualized plan of care
• Apply clinical knowledge and skills according to accepted standards of practice to facilitate problem resolution and achieve patient goals and outcomes
• Maintain the highest standards of professional conduct regarding confidential patient information

Required Qualifications

• Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Current nursing license in the practicing state
• Valid driver's license, auto insurance, and reliable transportation
• Current CPR certification
• One year of experience as an LPN/LVN in a clinical setting

Preferred Qualifications

• Prior experience in home health or hospice settings
