Licensed Practical Nurse (LPN) - Home Health - PRN

Enhabit Home Health & Hospice is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing services to patients in their homes on a PRN (as-needed) basis. In this role, you will administer intermittent professional nursing services, teach patients and families about their care, and work collaboratively with the healthcare team to implement physician-ordered plans of care.

Responsibilities

• Administer skilled nursing services to patients requiring intermittent professional care
• Teach patients, families, and healthcare team members about care plans and health management
• Perform nursing services in accordance with physician orders and established plans of care
• Provide care in patients' homes and coordinate with the interdisciplinary care team

Required Qualifications

• Graduate of an approved school of clinical education for practical nursing
• Current state licensure as a Licensed Practical Nurse in the state where you practice
• Minimum one year of experience as a licensed nurse
• Valid state driver's license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive an automobile in all types of weather conditions
• Current CPR certification
• Basic demonstrated technology skills, including operation of a mobile device

Preferred Qualifications

• Medicare home health or hospice experience
• Experience as a licensed professional may be deferred with review and approval from the regional vice president of operations or regional director
