Elara Caring is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes. As an LPN, you will deliver high-quality care according to the Plan of Care, physician's orders, and established clinical standards while working under the direction of a Registered Nurse or physician. You will care for patients across various stages of their health journey, helping them achieve their care goals and learn appropriate self-care techniques.

Clinical Setting

This is a PRN (as-needed) home health position requiring 50% travel. You will work in patients' homes, providing direct patient care and collaborating with the interdisciplinary care team.

Responsibilities

  • Verify the Plan of Care prior to each visit and provide care according to RN delegation, physician's orders, assessment data, and established standards
  • Incorporate patient care goals into nursing visits and document findings in skilled nursing notes
  • Communicate significant findings, problems, and changes in patient condition or environment to the RN Case Manager, physician, clinical supervisor, and other care team members
  • Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician
  • Implement patient safety plans using patient, family, and community resources
  • Assist patients in learning appropriate self-care techniques
  • Suggest revisions to the Plan of Care in response to identified patient care issues
  • Provide services requiring substantial and specialized nursing skills under RN or physician direction
  • Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions
  • Document medication regimens and update medication profiles according to established procedures
  • Treat patients, caregivers, and agency personnel with professionalism and respect
  • Implement actions to prevent avoidable hospitalizations and emergency room visits
  • Provide positive, supportive communication to physicians, patients, families, and agency personnel

Required Qualifications

  • Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
  • Current State License as a Licensed Vocational Nurse or Licensed Practical Nurse
  • One year of experience in a clinical care setting
  • Valid driver's license and insurance
  • Reliable transportation to perform job tasks
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
