Licensed Practical Nurse - LPN - Home Health - PRN

Life Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes across Texas, Florida, Nevada, and Georgia. In this PRN position, you will work under the direction of a registered nurse to deliver quality patient care, coordinate with interdisciplinary teams, and support patients and families in maintaining health and preventing illness.

Clinical Setting

You will work in patients' homes in various conditions, providing care to patients of all ages. The role requires reliable transportation and auto liability insurance, as you will travel locally to visit assigned patients.

Responsibilities

• Assist in identifying patients' physical, psychosocial, and environmental needs through documentation review, clinical records, case conferences, and team reports
• Plan and implement patient care in conjunction with the RN and in accordance with the Plan of Care
• Assist patients in recovery or maintenance of life functions and prevent illness and relapse
• Provide care utilizing infection control measures that protect both patient and staff in compliance with OSHA standards
• Ensure continuity of care through delivery of quality patient care
• Communicate effectively with patients, families, team members, and other healthcare professionals
• Participate regularly in case conferences and team meetings
• Monitor assigned cases to ensure compliance with third-party payer requirements
• Complete documentation in a timely manner in accordance with company policy
• Participate in in-service programs and maintain or improve clinical competency
• Meet mandatory continuing education requirements of the agency and licensing board
• Promote and exemplify the company's mission, vision, and values

Required Qualifications

• Completion of an accredited Licensed Vocational Nurse or Licensed Practical Nurse program
• Current LVN/LPN license in good standing in any applicable state
• Current CPR certificate
• Current driver's license in good standing
• One year of experience as an LVN/LPN in a clinical care setting
• Ability to read, write, and comprehend English
• Hepatitis profile if required to make patient visits
• Nursing skills as defined by generally accepted standards of practice
• Excellent interpersonal skills and ability to communicate effectively
• Proven decision-making skills

• Home health experience

• Prolonged standing and walking
• Ability to lift and transfer patients up to 50 lbs
• Hand-eye coordination and manual dexterity
• Ability to meet deadlines and patient needs, make quick decisions, and allocate resources under stressful conditions
• Ability to meet patient and family individualized psychosocial needs
