Licensed Practical Nurse (LPN) Home Health

Preferred Care Home Health Services is seeking Licensed Practical Nurses (LPNs) to provide skilled nursing care to patients in their homes. You will work under the direction and supervision of a Registered Nurse, delivering patient-centered care according to physicians' plans of care.

This full-time position offers the opportunity to build meaningful patient relationships while working alongside a supportive interdisciplinary team of nurses, therapists, and other healthcare professionals.

Responsibilities

• Provide skilled nursing services in accordance with the physician's plan of care and home health certification
• Administer medications and treatments as ordered
• Monitor patients for changes in condition and promptly communicate significant findings to the supervising RN and physician
• Accurately document patient care, clinical findings, treatments, and interventions in the medical record
• Prepare timely and complete clinical notes in accordance with agency and regulatory requirements
• Provide patient and caregiver education regarding medications, treatments, disease processes, and appropriate care techniques
• Assist with the development and evaluation of the patient's plan of care in collaboration with the patient, caregiver, representative, and interdisciplinary team
• Communicate effectively with physicians and other healthcare professionals involved in the patient's care
• Participate in case conferences, utilization reviews, orientations, in-services, and continuing education
• Participate in the agency's Quality Assurance and Performance Improvement (QAPI) program
• Maintain compliance with agency policies, procedures, regulatory requirements, and Code of Conduct
• Perform other duties as assigned

Required Qualifications

• Current and unrestricted Florida LPN license
• Graduate of an approved practical nursing program
• Current CPR certification
• Valid driver's license, reliable transportation, and appropriate auto insurance
• Strong clinical assessment, communication, organization, and documentation skills
• Ability to work independently while collaborating effectively with an interdisciplinary care team

Preferred Qualifications

• Previous home health experience
