Licensed Practical Nurse (LPN) Home Health

Phoenix Home Health is seeking a Licensed Practical Nurse (LPN) to provide home health nursing care in Sedalia, MO and surrounding areas. As an LPN, you will serve as the vital link between patients and their medical providers, executing tailored care plans based on physician orders while incorporating insights from initial assessments. You will work collaboratively with an interdisciplinary team including fellow nurses, Physical Therapists, Speech Therapists, and Occupational Therapists. A key advantage of this position is that charting is completed on-site with the assistance of Roger's AI-powered support, allowing you to maintain work-life balance.

Schedule

• Full-time, Monday through Friday, 8:00 AM to 5:00 PM
• Weekend rotation required every 4 to 6 weeks

Responsibilities

• Assist in review and revision of the plan of care
• Provide nursing care in accordance with the physician's plan of treatment and individual plan of care as authorized by the client's payer source
• Observe, record, and report reactions to treatment and any changes in the client's condition to appropriate personnel and/or physician
• Provide ongoing instruction to the client or family regarding the plan of treatment
• Participate in education programs, quality assessment activities, nurse meetings, and other staff meetings as required

Required Qualifications

• Missouri or Compact Registered Nursing or Licensed Practical Nursing License
• Strong organizational and self-management skills
• Strong and compassionate customer service skills
• Valid driver's license

Compensation and Benefits

• Pay range: $50,000 - $80,000
