Life Care Home Health is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes across Texas, Florida, Nevada, and Georgia. In this full-time role, you will work under the supervision of a registered nurse to deliver quality patient care, coordinate with interdisciplinary teams, and support patients in their recovery and maintenance of health.
Clinical Setting
You will work in patients' homes in various conditions, with possible exposure to blood-borne pathogens, bodily fluids, and infectious diseases. You may encounter unpleasant weather and may be required to respond to PRN emergency calls. You must have reliable transportation and auto liability insurance.
Responsibilities
- Administer nursing care under the direction of a registered nurse
- Assess clients' physical, psychosocial, and environmental needs
- Participate in planning and implementing care in accordance with the Plan of Care
- Assist clients in recovery or maintenance of life functions and prevent illness relapse
- Provide care utilizing infection control measures that protect both client and staff (OSHA compliance)
- Ensure continuity of care through delivery of quality patient care
- Communicate effectively with clients, families, team members, and other healthcare professionals
- Participate in case conferences and team meetings
- Monitor assigned cases to ensure compliance with third-party payer requirements
- Complete documentation in a timely manner to ensure compliance with company policy
- Participate in in-service programs and maintain professional competency
- Meet mandatory continuing education requirements of the agency and licensing board
Required Qualifications
- One year of experience as an LVN/LPN in a clinical care setting
- Completion of an accredited Licensed Vocational/Licensed Practical Nursing program
- Current LVN/LPN license in good standing in applicable states
- Current CPR certificate
- Current driver's license in good standing
- Ability to read, write, and comprehend English
- Hepatitis profile if required to make visits
- Nursing skills as defined by generally accepted standards of practice
- Excellent interpersonal skills and ability to communicate effectively
- Proven decision-making skills

Licensing and Certification Maintenance
Ongoing employment requires maintaining a valid LVN/LPN license in the state where you practice and a valid CPR certificate at all times. You are responsible for renewing both before they expire to continue employment.
