Licensed Practical Nurse (LPN) - Home Health

Enhabit Home Health & Hospice is seeking a Licensed Practical Nurse (LPN) to deliver skilled nursing services to patients in their homes. In this role, you will administer intermittent professional nursing care, teach patients and families about their care plans, and collaborate with the healthcare team to ensure quality outcomes aligned with physician orders and established plans of care.

Responsibilities

• Administer skilled nursing services to patients requiring intermittent professional care
• Teach patients, families, and healthcare team members about care plans and health management
• Perform nursing services in accordance with physician orders and the established plan of care
• Provide direct patient care in home-based settings

Required Qualifications

• Graduate of an approved school of clinical education
• Current state licensure as a Licensed Practical Nurse in the state where you practice
• Minimum one year of experience as a licensed nurse
• Valid state driver's license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive an automobile in all types of weather conditions
• Current CPR certification
• Basic demonstrated technology skills, including operation of a mobile device

Preferred Qualifications

• Medicare home health or hospice experience
• Experience as a licensed professional may be deferred with review and approval from the regional vice president of operations or regional director

Compensation and Benefits

• Hourly rate: $37.00-$40.00 per hour (based on years of experience)
• 30 paid days off annually, up to 6 weeks (includes company-observed holidays)
