Job Description

BAYADA Home Health Care is seeking a part-time Licensed Practical Nurse (LPN) to join our home health visit team. In this role, you will provide skilled nursing and rehabilitative care to clients in their homes, enabling them to maintain independence while receiving high-quality care. You will work as an integral member of a multidisciplinary healthcare team, traveling throughout the office's coverage area to deliver compassionate, reliable care.

Responsibilities

  • Perform comprehensive head-to-toe assessments with the ability to identify subtle changes in patient condition
  • Provide acute and post-acute care including management of conditions such as CHF, COPD, wound care, IV therapy, and post-surgical care
  • Triage and manage urgent clinical situations independently in the home setting
  • Perform medication reconciliation, patient teaching, and safety monitoring
  • Conduct comprehensive wound assessment, care, and documentation
  • Make real-time clinical decisions and escalate concerns appropriately
  • Manage a dynamic visit schedule and prioritize urgent visits
  • Complete point-of-care documentation efficiently using technology
  • Provide care in varied home environments while maintaining cultural sensitivity and respect for patient autonomy

Required Qualifications

  • Current nursing license in good standing in the state of Arizona
  • Graduation from an accredited and approved nursing program
  • At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population

Preferred Qualifications

  • Prior home health experience

Schedule and Travel

  • Part-time position with flexible scheduling
  • Must be able to travel throughout the office's entire coverage area
