AdventHealth is seeking a Licensed Practical Nurse (LPN) for Home Health to provide direct patient care in the South Volusia County area, including Oak Hill, South Daytona, Daytona, Daytona Beach Shores, Ponce Inlet, Port Orange, New Smyrna Beach, and Edgewater. This full-time position offers a Monday through Friday schedule from 8am to 5pm with flexibility, plus every third weekend. There is no on-call requirement.
Responsibilities
- Provide direct patient care for assigned patients in an efficient, organized manner under the direction of the RN
- Administer medications and perform treatments within your scope of training as ordered by the provider
- Perform medication administration, wound dressing changes, and ostomy care as directed by the RN and ordered by the physician
- Assess patients during clinical check-in, document pertinent medical information and vital signs, and assess for signs of abuse or neglect using age-specific guidelines
- Administer treatments, medications, and follow-up assessments ordered by the provider, verifying as needed
- Respond appropriately in clinical emergencies and triage situations
- Perform diagnostic and occupational medicine testing, including EKG, spirometry, audiometry, Titmus, and pulse oximetry
- Complete forensic testing collections in accordance with established guidelines
- Collect data specific to the age of the patient and contribute to their plan of care
- Receive, verify, and implement physician orders, ensuring orders taken by the LPN are co-signed by an RN
- Provide patient education as directed
- Report pertinent observations to the attending physician and RN, and implement changes in the patient care plan as directed
- Communicate changes to appropriate members of the home health care team
- Maintain clinical record information in accordance with established standards and within established time frames
- Participate in the review process and the agency performance improvement program
- Develop professional and clinical skills through attendance at staff development meetings, pertinent educational activities, and professional affiliations
- Display flexibility in accepting assignment changes and participate in weekend coverage
Required Qualifications
- Active Licensed Practical Nurse (LPN) license
- Basic Life Support CPR certification
- Valid driver's license
- Auto insurance
- High school diploma or equivalent
- Technical or vocational school training in LPN
- Venipuncture skills
- Proficiency in the English language
- Background check meeting organizational guidelines
- Ability to make emergency and mature independent judgments and decisions
- Excellent communication skills and ability to interact positively with associates
- High level of multitasking, organization, and proactive communication skills
- Problem-solving abilities
- Positive interpersonal communication skills
- Sound judgment and critical thinking skills

Schedule and Shift
- Full-time position

Compensation and Bonuses
- Pay range: $20.97 to $38.99 per hour
- Sign-on bonus up to $2,500 for eligible applicants
