Licensed Practical Nurse LPN (Home Care) North Palm Beach County

Actify Home Care is seeking Licensed Practical Nurses to provide home health care to geriatric patients across North Palm Beach County, including Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach, and surrounding areas. This is a 1099 independent contractor position with active cases ready for immediate assignment.

Responsibilities

• Provide high-quality, one-on-one patient care in patients' homes
• Implement and follow treatment plans established by the supervising RN
• Chart patient progress and treatment responses accurately and efficiently using Kinnser (WellSky)
• Coordinate and communicate patient status regularly with the supervising RN

Compensation

• Hi-Tech/Complex Care Visits: $35.00 per visit
• Routine Follow-up Visits: $30.00 per visit
• Reliable direct deposit every two weeks, processed automatically based on completed Kinnser visit logs

Required Qualifications

• Active Florida Licensed Practical Nurse (LPN) license
• Current CPR/BLS certification
• Valid driver's license and reliable transportation

Preferred Qualifications

• Kinnser/WellSky experience
