This is a full-time contract position for a Licensed Practical Nurse (LPN) providing direct and indirect nursing care to adult and geriatric residents in a long-term care setting. You will work under RN supervision, assessing and monitoring resident conditions, assisting with individualized care plans, administering treatments, and collaborating with the interdisciplinary team to ensure quality outcomes.
Responsibilities
- Provide direct and indirect nursing care to adult and geriatric residents under RN supervision
- Assess and monitor resident physical and psychosocial conditions and report changes to the RN and care team
- Assist in developing and updating individualized care plans
- Administer treatments and support daily care in accordance with facility policies and procedures
- Collaborate with the interdisciplinary team to ensure quality resident outcomes
- Maintain accurate documentation and ensure a safe, supportive care environment
Schedule
- Full-time position with 5 eight-hour evening shifts per week
- Every other weekend required
- Holidays required
Required Qualifications
- Active New York LPN license
- BLS certification (AHA or RQI only)
- Minimum one year of LPN experience in a long-term care setting
- High school diploma or equivalent
- Strong clinical, communication, and teamwork skills
