Licensed Practical Nurse LPN

Gurwin Healthcare System is seeking a Licensed Practical Nurse (LPN) for part-time positions across all shifts at our 460-bed nursing and rehabilitation center on Long Island. You will provide direct patient care to geriatric patients in a long-term care and subacute setting, working under the supervision and direction of registered nurses.

Clinical Setting

Gurwin Healthcare System is a comprehensive healthcare continuum serving the Long Island community, centered on a 36-acre campus in Commack, NY. The system includes a 460-bed nursing and rehabilitation center, assisted living, independent living, and home care agencies. This position is based at the main nursing and rehabilitation facility.

Position and Schedule

This is a part-time position available on all shifts:

• 7:00 AM - 3:15 PM
• 3:00 PM - 11:15 PM
• 11:00 PM - 7:15 AM

You will be scheduled every other weekend.

Responsibilities

• Participate in nursing care of patients within the scope of practical nursing practice, working under the supervision and direction of a registered nurse
• Participate in formulation and implementation of patient care plans to meet the specialized needs of geriatric patients under RN supervision
• Administer patient care as assigned by an RN within departmental guidelines
• Assume team leader, primary nurse, or charge responsibilities as needed under RN direction
• Direct and supervise non-professional staff as assigned
• Observe, record, and report patient data according to departmental policy
• Administer medications and perform nursing procedures as dictated by departmental policy
• Participate in unit nursing and interdisciplinary team activities
• Attend nursing and institutional in-service programs and participate in departmental committees as assigned
• Perform related and miscellaneous duties as directed; may be expected to perform unrelated duties in emergency situations

Required Qualifications

• Graduate of an accredited professional nursing program
• Current license to practice as an LPN in New York State
• Ability to establish effective interpersonal relations with patients, staff, and families
• Ability to communicate effectively in English, both orally and in writing
• Education in all areas of long-term care and subacute nursing to ensure competency on all units in the facility
• Demonstrates interest in the special needs of geriatric patients and ability to integrate principles of geriatric nursing into nursing practice

Compensation and Bonuses

• Sign-on bonus: $5,000 for part-time LPNs hired
• Rate increase to $36.19 - $39.81 after 6 months of employment
