Greendale Park Nursing & Rehab is seeking a Licensed Practical Nurse (LPN) to provide direct patient care to residents in our nursing and rehabilitation facility. You will work collaboratively with an interdisciplinary healthcare team to deliver individualized care and support to a diverse patient population.
Responsibilities
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with the healthcare team
- Monitor and report changes in patient condition to nursing staff and physicians
- Participate in discharge planning and education for patients and families
- Maintain accurate and thorough patient records
- Collaborate with interdisciplinary team members to ensure seamless care transitions
- Perform other duties as assigned by nursing staff and management
Required Qualifications
- Current licensure as a Licensed Practical Nurse (LPN) in Wisconsin
- High school diploma or equivalent
Preferred Qualifications
- 1-2 years of experience as an LPN
