Licensed Practical Nurse ( LPN )

We are seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents in our long-term care and rehabilitation facility. You will work as part of an interdisciplinary team, supporting residents' recovery and rehabilitation goals while maintaining the highest standards of compassionate, patient-centered care.

Clinical Setting

This is a long-term care facility specializing in rehabilitation services. You will work with residents recovering from illness or injury, collaborating with RNs, physicians, physical therapists, occupational therapists, speech therapists, and other healthcare professionals.

Responsibilities

• Administer medications and treatments as prescribed by physicians
• Monitor and document residents' health status, including vital signs, and report changes to the supervising RN or physician
• Assist with activities of daily living (ADLs), including bathing, dressing, and feeding, as needed
• Collaborate with the rehabilitation team to support residents' recovery goals, including physical, occupational, and speech therapy plans
• Maintain accurate and timely documentation in accordance with facility policies and state and federal regulations
• Provide emotional support and education to residents and their families
• Ensure a safe and clean environment, adhering to infection control protocols

Required Qualifications

• Current and valid LPN license
• BLS/CPR certification
• Strong clinical skills, including medication administration and wound care
• Excellent communication and interpersonal skills
• Ability to work effectively in a team-oriented environment
• Patient-focused approach to care
