Artis Senior Living is seeking a Licensed Practical Nurse (LPN) to serve as a Coordinator of Health & Wellness in our senior living community. In this full-time role, you will provide clinical monitoring and support for residents with Alzheimer's disease and dementia, ensuring all restorative, support, and preventative measures are met in accordance with state laws and regulations. You will work collaboratively with our care team to help residents achieve purpose, belonging, and joy while honoring their lifelong identity.

Clinical Setting

This position is based in a senior living community specializing in care for residents with Alzheimer's disease and dementia. You will work with an interdisciplinary team dedicated to person-centered care.

Responsibilities

  • Take an active role in resident assessments, development of service plans, and daily care
  • Oversee and implement the self-administered medication management program for residents
  • Administer medication and treatment to residents in accordance with physician orders
  • Document and report care provided and resident observations, including unusual or significant changes in physical, mental, or behavioral conditions
  • Provide emotional support, physical comfort, and ensure ongoing safety of residents
  • Supervise Caregivers and CNAs in providing resident care in accordance with state, federal, and company guidelines
  • Ensure residents receive the highest quality of service in a caring and compassionate atmosphere that recognizes their individual needs and rights
  • Assist in marketing the community by touring potential residents and participating in public relations events
  • Assist with resident move-ins and relocations, helping residents acclimate to their new surroundings and community personnel
  • Perform other duties as required

Schedule

  • Full-time position
  • First shift (7 AM - 3 PM) and second shift (3 PM - 11 PM) available
  • Weekend availability required

Required Qualifications

  • Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing
  • Fulfill and maintain continuing education credits as required by state
  • Experience with Alzheimer's disease, dementia, or behavioral health conditions
  • Willingness to work with the elderly population

Preferred Qualifications

  • Experience in rehabilitative or geriatric nursing
