Licensed Practical Nurse (LPN)

FEHE Enterprises, Inc. is seeking Licensed Practical Nurses to provide nursing care and healthcare support to individuals with intellectual and developmental disabilities in residential settings. You will work under the direction of the Director of Nursing or supervising RN, providing direct care, medication administration, health monitoring, and clinical documentation while practicing within the Georgia LPN scope of practice.

Clinical Setting

You will work in FEHE residential homes and community-based service locations, providing nursing coverage between 7:00 a.m. and 8:00 p.m. daily. The role involves travel among multiple FEHE residential homes and program locations as assigned.

Responsibilities

• Provide safe and compassionate nursing services in accordance with physician orders, healthcare plans, nursing protocols, and applicable scope-of-practice requirements
• Monitor vital signs, pain, skin integrity, wounds, nutrition, hydration, elimination, seizures, falls, mobility, behavior, and other identified health risks
• Identify and promptly report changes in an individual's condition to the Director of Nursing, supervising RN, physician, or appropriate program leader
• Administer prescribed medications, enteral feedings, treatments, first aid, and other authorized nursing interventions accurately and safely
• Monitor individuals for medication side effects, adverse reactions, refusals, omissions, or other medication-related concerns
• Review medication administration records, physician orders, pharmacy labels, controlled-medication records, and related documentation for accuracy and completeness
• Conduct routine medication-cart, medication-room, and clinical-record audits
• Coordinate prescription refills, pharmacy communication, medication deliveries, new and discontinued orders, prior authorizations, and provider clarification
• Coordinate and track medical appointments, laboratory work, referrals, hospital or urgent-care visits, discharge instructions, and provider recommendations through completion
• Assist with maintaining accurate Health Risk Screening Tool records and report information that may require an HRST review or update
• Maintain timely and accurate nursing notes, treatment records, medication documentation, appointment records, tracking logs, audit reports, and follow-up lists
• Provide clear healthcare instructions to direct-support professionals and program staff regarding medications, health protocols, infection control, vital signs, dietary needs, positioning, mobility, and emergency response
• Reinforce staff training and healthcare expectations when assigned and authorized
• Respond appropriately to medical emergencies, injuries, medication errors, hospital visits, and other reportable incidents
• Participate in nursing meetings, individual service-plan meetings, incident reviews, quality-improvement activities, chart reviews, and regulatory audits
• Maintain adequate clinical supplies and equipment, monitor expiration dates, and report supply or equipment concerns
• Protect individual rights, dignity, privacy, and confidential health information at all times
• Follow FEHE policies and all applicable DBHDD, DCH, CARF, HIPAA, infection-control, documentation, and safety requirements
• Perform other related nursing and administrative duties as assigned

Required Qualifications

• Active and unrestricted Licensed Practical Nurse license in the State of Georgia
• At least one year of nursing experience
• Current CPR and First Aid certification or the ability to obtain required certifications within the established timeframe
• Working knowledge of nursing procedures, medication administration, clinical documentation, and health-monitoring practices
• Ability to recognize and report changes in an individual's health or functioning
• Ability to work independently while maintaining regular communication with the Director of Nursing, supervising RN, and program leadership
• Strong written, verbal, organizational, and time-management skills
• Valid driver's license, acceptable motor-vehicle record, and reliable transportation
• Ability to travel among FEHE locations when assigned
• Ability to successfully complete all required background checks, fingerprinting, screenings, training, and competency requirements

Preferred Qualifications

• Previous nursing experience supporting individuals with intellectual and developmental disabilities
• Experience in a group home, residential-services, community-based, long-term care, behavioral health, or home-care setting
• Experience with medication administration, medication audits, physician orders, pharmacy coordination, and appointment follow-up
• Familiarity with HRST, Therap, electronic health records, or other clinical documentation systems
• Experience providing healthcare guidance or training to direct-support professionals
• Knowledge of person-centered care, individual healthcare plans, behavior-support plans, and IDD-related health risks

Physical Requirements

• Ability to stand, walk, sit, bend, reach, and remain physically active throughout the assigned shift
• Ability to assist individuals with mobility, positioning, transfers, personal care, and emergency response when required
• Ability to lift, carry, push, or pull supplies and equipment associated with residential nursing care
• Ability to perform the essential functions of the position safely, with or without reasonable accommodation

Schedule and Employment

• Full-time and part-time opportunities available
• Flexible scheduling based on program needs within the 7:00 a.m. to 8:00 p.m. daily coverage period
• Specific shifts and days will be discussed based on candidate availability and organizational needs
• Some weekend and holiday availability may be required

Compensation

• Pay: $36.00 to $38.00 per hour
