Edna Tina Wilson Living Center, a CMS 5-Star long-term and post-acute care facility in Rochester, is seeking a Licensed Practical Nurse (LPN) for a per diem position. You will provide professional patient care to residents while directing and monitoring care provided by CNAs and ensuring proper documentation. Responsibilities
- Administer medications to assigned residents in a timely and accurate manner with concurrent required documentation
- Perform treatments and document progress according to physician orders and facility protocol
- Ensure concise, pertinent, and complete resident care documentation using the computerized medical record system and initiate appropriate care pathway tools
- Direct assigned CNAs in timely completion of quality individualized personal care in compliance with residents' individual care plans and ensure CNA documentation is accurate and complete
- Receive and give relevant reports, both written and verbal, to appropriate staff according to departmental protocols
- Contact physicians, transcribe orders, and follow up with appropriate action to institute orders in a timely manner
- Monitor residents and report relevant condition status changes to the charge nurse, supervisor, or physicians in a timely manner
- Complete pre-reviews of medication administration records according to facility protocols
- Complete required MDS documentation and all related observations in a timely manner
- Support the CNA team by assisting in direct activities of daily living (ADL) care for residents
Required Qualifications
- Graduation from an accredited School of Practical Nursing
- Current LPN licensure in the State of New York
- Basic Life Support (BLS) Certification from the American Heart Association (AHA)

- One year of experience
Schedule
- Per diem position with flexible scheduling
- Must complete a minimum of 40 hours per month, including 16 weekend hours
- Various shifts available
Physical Requirements
- Medium work requiring exertion of 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
- Frequent walking, standing, or squatting required
Compensation
- Pay range: $28.51 to $34.68 per hour
- Base pay determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts
- Eligible for additional pay components
