This position offers Licensed Practical Nurses the opportunity to provide direct clinical care to underserved and vulnerable patient populations in a mission-driven healthcare organization. Working under the supervision of RNs, you will contribute to comprehensive patient assessment, treatment planning, and implementation across both health care units and housing units.
Clinical Setting
You will work in a healthcare environment serving hundreds of thousands of unique individuals monthly, many of whom receive direct clinical care in facilities operated by Wellpath. The organization emphasizes non-judgmental compassion and evidence-based care for society's most vulnerable populations.
Responsibilities
- Assess patients' physical, psychological, and social dimensions under RN supervision, contributing to individualized treatment planning and consulting with RNs and staff as needed
- Implement treatment programs directed by Health Care Practitioners, including administering medications and IV therapy (if certified) and performing diagnostic tests according to established protocols
- Collaborate in controlled substance counts at the beginning of each shift, adhering to safety techniques, procedures, and standard precautions
- Respond to codes or health emergencies within standard guidelines
- Assist in medical or minor surgical procedures
- Implement nutrition and therapeutic diet plans
- Document nursing encounters using SOAP charting and ensure effective communication with nursing staff, physicians, and other relevant personnel
- Attend mandatory staff meetings and training sessions, demonstrating commitment to continuous professional development and adherence to policies and procedures

Required Qualifications
- Graduate from an accredited school of nursing
- Current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse within the state of employment
- Ability to obtain and maintain CPR certification
- Reside in the United States
