Licensed Practical Nurse (LPN) Days

Countryside Manor is seeking a full-time Licensed Practical Nurse (LPN) for day shift in Fremont, Ohio. You will provide direct nursing care to residents in a skilled nursing and long-term care setting, working with an experienced and supportive nursing team.

Clinical Setting

Countryside Manor is a skilled nursing and long-term care facility. You will care for residents in this setting, working collaboratively with CNAs and other members of the nursing team.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and long-term care setting
• Administer medications and treatments as ordered
• Monitor residents and assess changes in condition
• Document resident care accurately and timely
• Communicate changes in resident condition to physicians and appropriate members of the care team
• Follow individualized resident care plans
• Provide guidance and support to CNAs and other members of the nursing team
• Respond promptly to resident needs and concerns
• Maintain resident dignity, privacy, comfort, and safety
• Follow infection control, safety, and facility procedures
• Communicate effectively with residents, families, physicians, and healthcare team members

Required Qualifications

• Current Licensed Practical Nurse (LPN) license
• Compassionate, dependable, and resident-focused approach
• Strong clinical, communication, and teamwork skills

Preferred Qualifications

• Long-term care, nursing home, or skilled nursing experience
