Licensed Practical Nurse (LPN Day Hab)

Bridgewell is seeking a Licensed Practical Nurse (LPN) to provide healthcare services within our Day Habilitation program. Under the supervision of the Healthcare Coordinator, you will assist in coordinating medical services, delivering direct healthcare, and managing medical documentation for individuals with disabilities in a community-based day program setting.

Clinical Setting

This position is based within Bridgewell's Day Habilitation Services, which serves individuals with disabilities and other life challenges. You will work in a community-based day program environment across Bridgewell's locations on the North Shore, Lowell, and Merrimack Valley.

Responsibilities

• Assist the Healthcare Coordinator in coordinating medical services with each recipient's primary care physician, medical clinic, or residence
• Provide healthcare services to all individuals in the program
• Assist in completing all required medical documentation, including seizure reports, medication and treatment charts, and other records
• Establish and oversee groups in relevant areas such as hygiene and gender-specific groups
• Provide monthly in-services and training to all staff regarding pertinent medical information, services, and communicable diseases
• Inform the clinical team of all medical changes, needs, and concerns
• Complete medical treatments as indicated
• Complete medication disbursement and all required documentation
• Purchase all necessary medical supplies
• Maintain all appropriate certifications
• Perform other duties as assigned

Schedule

Monday through Friday, either 8:00 AM to 4:00 PM (40 hours per week) or 9:00 AM to 4:00 PM (35 hours per week).

Required Qualifications

• High School Diploma or GED
• Current registration with the Massachusetts Board of Registration on Nursing
• One year of experience working with individuals with disabilities
• One year of experience in a community setting
• Knowledge of Day Habilitation Regulations and CARF standards
• Knowledge of behavior management and human rights
• Excellent organizational skills, problem-solving, and communication skills

Physical Demands

• Frequently required to climb stairs
• Ability to lift and/or move up to 20 pounds
• Normal range of eyesight with or without corrective lenses
• Normal range of hearing with or without corrective equipment

Additional Requirements

• Acceptable CORI (Criminal Offender Record Information) results according to Bridgewell policy
• Additional compensation is provided for on-call rotations
