Licensed Practical Nurse (LPN)- Day & Evening shifts

Pleasant Hills Community Living Center is seeking a Licensed Practical Nurse (LPN) to join our team in a long-term care setting. This full-time position offers day and evening shift options, providing direct patient care to residents in a community living center environment.

Schedule

• Full-time position
• Day shift: 7:00 AM to 3:00 PM
• Evening shift: 3:00 PM to 11:00 PM

Responsibilities

• Evaluate and deliver care including medication administration, treatments, and activities of daily living (ADL) assistance as ordered by the physician and outlined in the plan of care, within the scope of practice
• Conduct regular rounds to monitor resident activity and ensure residents receive quality care based on professional standards and facility policies
• Communicate with residents, family members, physicians, and supervisors regarding resident status
• Maintain up-to-date and accurate medical records during your tour of duty on the assigned unit
• Perform other tasks as required

Required Qualifications

• Licensed Practical Nurse (LPN) license in good standing
• Basic Life Support (BLS) certification
