Licensed Practical Nurse (LPN)

CVP is seeking Licensed Practical Nurses (LPNs) to provide skilled nursing care to Veterans at the VA New Jersey Health Care System (VANJHCS) at East Orange VA Medical Center in East Orange, NJ. You will work across a variety of clinical units, operating both independently and under the guidance of the Nurse Manager, delivering high-quality patient care to our Nation's Veterans.

Responsibilities

• Provide skilled nursing care to patients across multiple units
• Prepare, administer, and accurately document oral, topical, subcutaneous, intramuscular, and intravenous medications in accordance with facility policies and procedures
• Observe and document patient responses to medications and changes in patient condition, reporting findings to the supervising RN or physician in a timely manner
• Perform and document nursing assessments within required timeframes, including head-to-toe assessments and vital signs monitoring
• Identify and act on significant or predictable changes in patient health status, escalating concerns to appropriate clinical staff
• Maintain and update patient care plans to reflect current health status, interventions, and goals in alignment with the interdisciplinary care team
• Collaborate with physicians, registered nurses, and other members of the interdisciplinary healthcare team to ensure coordinated, high-quality patient care
• Accurately document all patient care activities, assessments, and clinical observations in the VA's Computerized Patient Record System (CPRS) electronic health record in accordance with VHA Handbook 1907.01 and facility documentation standards
• Maintain patient confidentiality and a safe care environment in accordance with HIPAA, the Privacy Act, and all applicable VA regulatory requirements, including those set forth by The Joint Commission (TJC)

Required Qualifications

• Graduate of a state-approved school of practical or vocational nursing
• Current, active, full, and unrestricted license to practice as a Licensed Practical Nurse (LPN) in any State, Territory, Commonwealth of the United States, or the District of Columbia
• Minimum of one year of clinical nursing experience as an LPN
• Current Basic Life Support (BLS) certification issued by the American Heart Association
• Familiarity with electronic health record systems
• Demonstrated ability to communicate effectively in spoken and written English, including fluency in reading and writing clinical documentation
• Eligible to obtain a government-sponsored background investigation
• US Citizenship is required

Preferred Qualifications

• Experience in medical/surgical, geriatric, or psychiatric nursing settings

Compensation

• Hourly Pay: $34/hr
• Health & Wellness (H&W): $5.09/hr (H&W will be prorated based on enrollment in the benefits offered, including automatic enrollment in employer-paid plans)
