Learning Services, a Collage Rehabilitation Partner, is seeking a Full-Time Licensed Practical Nurse (LPN) in Gilroy, CA. You will work as part of a clinical team providing nursing and facilitation services to adults with moderate to severe traumatic and acquired brain injuries. The organization specializes in treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues, and accident or sports-related head injuries. You will work under the direction of a Registered Nurse and implement evidence-based treatment strategies that promote physical, psychosocial, and spiritual health.
Responsibilities
- Utilize nursing knowledge and skills in the safe implementation of basic preventative, therapeutic, and rehabilitative nursing care for assigned clients
- Complete timely identification of individuals in need of medical nursing services or interventions and provide interventions as needed
- Perform hands-on nursing care and facilitator care using the nursing process
- Provide direct care to clients at program locations or during medical appointments as needed
- Train and educate clients, families, funders, employers, and other constituents
- Develop and implement individualized teaching and discharge plans using appropriate resources
Required Qualifications
- State licensure as a Licensed Practical Nurse (or equivalent)
- Valid driver's license with an acceptable driving record maintained at all times
- Demonstrated accountability and skills in analysis, problem solving, and decision making related to medical nursing skills and time management
- Ability to communicate in writing and verbally with families, clients, staff, and physicians
- Competence in writing, planning, organization, and documentation
