Clifton Springs Nursing Home is seeking a Licensed Practical Nurse (LPN) to provide professional patient care to long-term and post-acute care residents on the night shift. In this role, you will direct and monitor care provided by CNAs, ensure proper documentation, and serve as a clinical leader for your team.
Clinical Setting
This is a full-time position in long-term care at Clifton Springs Nursing Home, working 37.5 hours per week on the night shift (11pm-7am) with every other weekend required.
Responsibilities
- Administer medications to assigned residents in a timely manner and complete all required documentation
- Perform treatments and document progress according to physician orders and facility protocol
- Ensure concise, pertinent, and complete resident care documentation using the computerized medical record system and initiate appropriate care pathway tools
- Direct assigned CNAs in the timely completion of quality individualized personal care in compliance with residents' individual care plans
- Ensure CNA documentation is accurate and complete
- Receive and give relevant reports, both written and verbal, to appropriate staff according to departmental protocols
- Contact physicians, transcribe orders, and follow up with appropriate action to institute orders in a timely manner
- Monitor residents and report relevant condition status changes to the charge nurse, supervisor, or physicians in a timely manner
- Complete pre-reviews of medication administration records according to facility protocols
- Complete required MDS documentation and all related observations in a timely manner
- Support the CNA team by assisting with direct activities of daily living (ADL) care for residents
Required Qualifications
- Current New York State Licensed Practical Nurse license
- Graduate from an accredited School of Practical Nursing or hold an ASN/BSN in Nursing
- BLS (Basic Life Support) certification from the American Heart Association
Preferred Qualifications
- One year of nursing experience
Physical Requirements
This position requires medium work, exerting 20 to 50 pounds of force occasionally and/or 10 to 25 pounds of force frequently. Frequent walking, standing, or squatting is required.
