Licensed Practical Nurse (LPN)

Chalet Rehabilitation & Nursing is seeking a Licensed Practical Nurse (LPN) to provide direct nursing care to residents and supervise day-to-day nursing activities performed by nursing assistants. You will work in a long-term care setting, ensuring the highest degree of quality care in accordance with federal, state, and local standards.

Responsibilities

• Direct the day-to-day functions of nursing assistants and provide leadership to nursing personnel assigned to your unit or shift
• Meet with assigned nursing staff and support personnel to plan shift services, programs, and activities
• Make written and oral reports and recommendations concerning shift activities as required
• Admit, transfer, and discharge residents as required
• Complete and file required record-keeping forms and charts upon resident admission, transfer, and discharge
• Receive and transcribe telephone orders from physicians and record on the Physician's Order Form
• Chart nurse's notes in an informative and descriptive manner reflecting care provided and resident response
• Fill out and complete accident and incident reports and submit to the Director as required
• Perform routine charting duties in accordance with established policies and procedures
• Prepare and administer medications as ordered by the physician
• Administer professional nursing services including catheterization, tube feedings, suction, applying and changing dressings and bandages, colostomy and drainage bag care, blood draws, massages, range of motion exercises, and care for the dying
• Develop and maintain good working rapport with interdepartmental personnel to ensure nursing services meet resident needs
• Create and maintain an atmosphere of warmth, personal interest, and positive emphasis throughout the unit
• Receive and give nursing reports at the beginning and end of shift
• Make rounds with physicians as necessary
• Monitor assigned personnel to ensure they follow established safety regulations
• Maintain confidentiality of all resident care information and protected health information
• Report known or suspected incidents of unauthorized disclosure of protected health information
• Ensure all nursing care is provided in privacy
• Perform other duties as assigned

Required Qualifications

• Current unencumbered Licensed Practical Nurse state license
• Ability to read, write, speak, and understand the English language
• Ability to make independent decisions when circumstances warrant
• Knowledge of nursing and medical practices, procedures, laws, regulations, and guidelines pertaining to nursing care facilities
• Current CPR Certification

• Ability to push, pull, move, and/or carry such weight a minimum distance of 50 feet
