Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse (LPN) to provide direct patient care in our facility. This role involves conducting patient assessments, managing admissions and discharges, providing wound care, and leading certified nursing assistants and medication technicians. You will respond to emergency situations and ensure compliance with facility protocols and policies.

Responsibilities

• Conduct patient assessments, follow established protocols, and report medical or behavioral changes
• Document clinical findings and maintain accurate patient records
• Complete admission duties including assessments, medication administration records (MAR), treatment administration records (TAR), plan of service (POS), chart setup review, and physician order processing
• Manage patient discharges by completing required documentation, notifying physicians and guardians, and arranging transportation
• Provide wound care including irrigation, packing, and dressing removal according to established protocols
• Lead and supervise CNAs and CMTs to ensure compliance with facility protocols and policies
• Respond effectively to emergency situations

Required Qualifications

• Active LPN license
