Licensed Practical Nurse (LPN)

Blue Palms Health & Rehabilitation of DeLand is seeking Licensed Practical Nurses to join our healthcare team in a collaborative, fast-paced environment. This position offers the opportunity to provide direct patient care while developing your clinical skills and making a meaningful impact on patient outcomes.

Clinical Setting

This role is based in a health and rehabilitation facility setting where you will work with patients requiring ongoing medical care and support.

Responsibilities

• Provide basic medical care including measuring vital signs, administering medications, and assisting with personal hygiene tasks
• Monitor and record patients' health status and report any changes to the nursing staff
• Perform routine procedures, including dressing changes and injections, under supervision
• Maintain accurate patient records, documenting care provided and patient responses to treatments
• Collaborate with multidisciplinary teams to ensure comprehensive patient care
• Educate patients and their families about health management and disease prevention

Required Qualifications

• Current and valid Licensed Practical Nurse (LPN) license in the state of Florida
• Current CPR or BLS Card
• Strong understanding of nursing principles and practices, as well as applicable regulations
• Excellent verbal and written communication skills for interacting with patients and team members
• Ability to work in a fast-paced environment and manage multiple tasks effectively
• Basic computer skills for maintaining electronic health records and documentation
