Licensed Practical Nurse (LPN)-Bartram Memory Care-Full-Time

Brooks Rehabilitation is seeking a Licensed Practical Nurse (LPN) for Bartram Memory Care, a specialized memory care community serving elders with advanced dementia. This full-time position is located at a GREEN HOUSE® Residence on the Bartram Campus in Jacksonville, Florida—a state-of-the-art, home-like setting on 115 acres of green space designed specifically for elder care.

As an LPN, you will work under the direction of a registered nurse to provide direct patient care, participate in care planning, and support the specialized needs of residents with advanced dementia. You will be part of a dedicated 24-hour nursing team serving as care partners with specialized knowledge and skills for this population.

Responsibilities

• Document observations and care provided according to company policies and procedures
• Administer medications
• Provide basic nursing care measures and assist with care for residents
• Admit, transfer, and discharge residents
• Observe, record, and report subjective patient symptoms
• Assist the RN in providing physical and emotional support to residents
• Manage dietary and fluid needs, including nourishment, feeding, tube feeding, and IV fluid observation
• Record and maintain accurate fluid intake and output
• Take and record vital signs: temperature, pulse, respirations, and blood pressure

Required Qualifications

• Active Florida LPN license
• One year of senior care, short-term respite care, or skilled nursing experience
• Current hands-on BLS certification

Preferred Qualifications

• IV certification

Schedule

• Full-time, 3:00 PM to 11:00 PM shift
