Licensed Practical Nurse (LPN)

Award Care is seeking Licensed Practical Nurses (LPNs) to provide direct patient care in our skilled nursing and rehabilitation facility. In this role, you will assist residents and patients with activities of daily living, implement individualized treatment plans, and work under the direction and supervision of professional nurses. New graduates are welcome to apply.

Clinical Setting

You will work in a health and rehabilitation center (skilled nursing home) serving residents requiring skilled nursing care and rehabilitation services.

Responsibilities

• Provide direct patient care to assigned residents and patients
• Assist in implementing individualized treatment plans for each assigned resident
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
• Oversee the Falls Program and Pressure Sore Program
• Conduct and document thorough evaluations of each resident's medical status upon admission and throughout their course of treatment
• Perform other duties as assigned

Required Qualifications

• Current, unencumbered, active state license to practice as an LPN

Preferred Qualifications

• Experience in skilled nursing or rehabilitation facilities
