Licensed Practical Nurse, LPN

Aviata at Santa Barbara in Cape Coral, Florida is seeking a Licensed Practical Nurse (LPN) to provide direct patient care to residents in a skilled nursing facility. You will assist residents with activities of daily living, implement individualized treatment plans, and work under the direction and supervision of a professional nurse as part of a collaborative care team.

Responsibilities

• Assist in implementing individualized treatment plans for assigned residents
• Supervise Certified Nursing Assistants (CNAs) and participate in their evaluation
• Oversee the Falls Program and Pressure Sore Program
• Provide nursing services
• Conduct and document thorough evaluations of each resident's medical status upon admission and throughout their course of treatment
• Perform other duties as assigned

Required Qualifications

• Current, unencumbered, active state license to practice as an LPN

Preferred Qualifications

• Experience in skilled nursing or rehabilitation facilities

Schedule

• 12-hour shifts
• 3 days on, 4 days off
