Licensed Practical Nurse (LPN) Assisted Living!

Independence Village Avon Lake is seeking a Licensed Practical Nurse (LPN) to serve as a Nurse Supervisor in our assisted living community. In this role, you will lead and manage a team of caregivers while providing clinical oversight to our residents. You will evaluate residents, report changes in condition to physicians, and utilize Electronic Health Records (EHR) and Electronic Medication Administration Records (EMAR) to support quality care.

Responsibilities

• Lead, manage, and train your team of caregivers
• Evaluate residents and report changes in condition to physicians
• Utilize Electronic Health Record (EHR) and Electronic Medication Administration Record (EMAR) systems
• Communicate effectively with team members, residents, and families
• Build and maintain strong relationships with residents and their families

Required Qualifications

• Active Licensed Practical Nurse (LPN) license in the state
• Prior supervisory experience required
• Demonstrated punctuality and professionalism

Preferred Qualifications

• Assisted Living or Memory Care experience
• Passion for working with elderly populations
