Arete at Desert Sage Memory Care is seeking a Licensed Practical Nurse (LPN) to provide direct medical care and clinical leadership in a memory care and assisted living facility located in Bend, Oregon. Working under the supervision of a Registered Nurse, you will manage medications, assess resident health, oversee frontline caregivers, and communicate with families and physicians. This role is ideal for an LPN with experience in dementia and memory care who is ready to make a meaningful impact on seniors' lives.
Clinical Setting
Arete at Desert Sage Memory Care is a memory care and assisted living facility specializing in care for residents with Alzheimer's disease and dementia. The facility serves seniors requiring cognitive and physical support in a long-term care environment.
Responsibilities
- Safely prepare, administer, and document medications and treatments according to pharmacy and state regulations
- Conduct regular narcotic counts and ensure compliance with all regulatory requirements
- Assess residents' conditions upon move-in and complete ongoing, routine health evaluations
- Monitor vital signs and track changes in physical health or cognitive behaviors
- Tailor care to residents with Alzheimer's or dementia, providing redirection and managing memory-related behavioral challenges
- Offer structured cognitive support aligned with residents' needs
- Follow Individualized Service Plans (ISPs) designed by the wellness director or RN, adjusting care as resident needs evolve
- Supervise Certified Nursing Assistants (CNAs), Caregivers, Medication Aides/Techs, and care partners, offering guidance and assistance during daily routines
- Communicate with physicians regarding orders and updates
- Provide compassionate updates to families and respond to their concerns
- Conduct thorough assessments of new and existing residents to identify healthcare needs, preferences, and goals
- Continuously update care plans based on changes in health status
- Educate resident assistants and medication technicians on dementia-specific best practices, infection control, and emergency procedures
- Ensure staff are properly following structured daily routines critical for lowering stress and supporting memory care residents
- Provide emotional and psychosocial support to residents and their families, connecting them to community resources
- Handle dynamic and evolving healthcare situations by applying critical thinking skills and effectively responding to emergencies both in person and remotely
- Address changes in resident health and staff challenges as they arise
Required Qualifications
- Current, unencumbered, active License to Practice as a Licensed Practical Nurse in Oregon
- Nursing degree from an accredited college
- Nursing experience in assisted living, Alzheimer's/dementia, long-term care, geriatric rehabilitation, sub-acute care, or critical care clinical settings
- Thorough knowledge of CMS and State Regulations regarding patient care in assisted living facilities (ALF), independent living (IL), and memory care (MC) facilities
- Experience with electronic medical records (EMR) documentation
- Strong leadership, mentorship, and communication skills
- Knowledge of State and Federal regulations and compliance requirements
- Excellent organizational and time management skills
- Active CPR/BLS certification during employment
- Ability to maintain confidentiality of all resident care information
- Ability to create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment
